Random Vaccination Attestation Audits

On Tuesday, April 19, Seattle Colleges will begin a pilot program at all three colleges to conduct random audits to verify the vaccination attestation of students attending on-campus (or hybrid with an on-campus component) instruction. The goal is to ensure the health and safety of our campus community, and we will make it as easy as possible for students to complete.

A small, randomly selected group of students who have already attested that they are vaccinated will be asked to upload their CDC vaccination cards or other official proof of vaccination for verification using a secure online form. Once the information has been verified, it will be purged from the system.

This step is different than attesting in ctcLink; this is a request to verify a student’s vaccination status by having them show their CDC vaccination card or other form of proof. Students will be notified of their selection by email and text.

Students will have until Tuesday, May 3, to complete this verification process.

If you are selected to complete a random audit of your vaccination attestation: 

  • You will receive an email and text message. 
  • Follow the web link in that message and complete the secure online form. 
  • Have your CDC COVID-19 vaccination card or other official proof of vaccination ready. Take a photo of the document with your smart phone or your computer’s web cam so that the writing is clearly visible. You may also use waverify.doh.wa.gov to get a digital copy of your COVID-19 vaccination record. Upload that photo or PDF to the online form. 
  • Submit the completed online form by Tuesday, May 3, 2022.

This is a continuation of our COVID-19 protocols to ensure the health and safety of our campus community. Faculty and staff have already verified their vaccination status by showing their CDC cards to Human Resources. We are extending this verification process to a small group of randomly selected students.

Note: the governor's Higher Education Proclamation still requires all students to attest as to their vaccination status or file an exemption

A small, randomly selected group of students registered for hybrid or in-person instruction at each college will be asked to verify their vaccination attestation.

  • Online-only students
  • Students who have claimed a religious or health exemption

A list of student ID numbers will be randomly generated by computer.

Students selected for the verification process will receive a link to a secure online form where they can upload their proof of vaccination documents.

Yes. The web form will be a secure form to maintain your privacy. Your private information will not be shared or stored. Once your information has been verified, it will be purged from the system.
